How to get a Library Card

Any Oak Lawn resident 16 years of age or older can sign-up for a Library card. If your photo ID has your Oak Lawn address on it that’s all you need! If your ID does not have your Oak Lawn address, you will need one of the following with your photo ID:

  • A utility bill
  • Motor vehicle registration card
  • Property transfer certificate, lease or mortgage papers
  • Voter registration card
  • Senior citizen membership card, class schedule, Public Aid card or medical aid ID card
  • Envelope with a postal change of address label and current postmark
  • Pre-printed checks
  • Any pre-printed form of a current bill with name and address

Legal guardians must sign the application acknowledging responsibility for library materials for minors under 16 who wish to get a card. Business and property owners may apply for a card by presenting proper identification and a current tax bill or a letter on the business’ stationery requesting such service, signed by a business owner or officer. Only one card may be issued to a property or business owner and cards are good for one year. Nonresidents may purchase an OLPL card at a price determined annually by the Board of Trustees. Please report all lost or stolen cards as soon as possible so that we may block the card and prevent anyone else from using it.

This card registration is for a full library card which you can use to check out materials from any of the 100 SWAN libraries and use for any of the digital resources available through the Oak Lawn Public Library, including eBooks, eAudiobooks, and online databases. Please fill out the application below, or download the form and mail the it to the library. We will mail your library card to you. We ask that you prove your residency within 90 days in-person at the library with a government issued photo ID with your Oak Lawn address on it. Need multiple applications?
